Pre-Adoption Application
These should be people who have observed you with your previous pet(s) or can speak to your character if you have never owned a pet before Only one of these references may be a relative. If for some reason we are unable to reach these people we may contact you for more information or another reference *
Adoption fee for dogs is $150 and $100 for kittens, $50 for cats. All animals are spayed/neutered, up to date on age appropriate vaccinations, microchipped, heartworm tested/up to date on preventative (for dogs) and feline leukemia/FIV tested (for cats).
If FOPAS should be made aware that a dog adopted from us is at a shelter or other holding facility ("other holding facility" to include but not limited to a veterinarian's office) for more than 48 hours and you have not notified FOPAS that the pet is missing, FOPAS has the right but not the obligation to reclaim the dog as its own.* Your signature below is confirmation that you understand this and agree to these terms.
By submitting this application I certify that the information I have given is true and that I recognize that any misrepresentation of facts may result in my losing the privilege of adopting a pet and nullification of any contract in the future I understand that the Friends of Parkville Animal Shelter have the right to deny my request to adopt an animal and I authorize investigation of all statements in this application I understand that this application is the property of Friends of Parkville Animal Shelter.
