WAIVER: MUST BE ELECTRONICALLY SIGNED BY PARTICIPANT AND OWNER OF DOG FOR PARTICIPATION.

This Waiver, Release and Covenant Not to Sue and Indemnification of Agreement must be signed by the Paws in the Park Event Participant, the owner of the dog participant, and the parent/guardian of any participant under the age of 18 in order to participate. I give and agree to the following Agreement in one or more of the following capacities and for and on behalf of the following: 1) For myself in all cases, whether or not a participant, 2) for my child or ward if a participant in the event and 3) for my dog. I understand the following warnings and disclosures: (i) no person or dog should participate unless physically able and properly trained; (ii) all participants and dogs assume any and all risks associated in participating in the event, including, but not limited to, falls, contact with other participants, motor vehicles, dog bite, tripping, the effects of weather and location conditions. I hereby waive, for and on behalf of myself, my dog, and my child/ward participant if under 18 years of age, and release and forever discharge and covenant not to sue, and further agree to hold harmless and indemnify Friends of Parkville Animal Shelter, its officers, members, sponsors, volunteers, the city of Parkville, the city of Kansas City, their representatives, their agents, employees, assigns or others acting on their behalf from any and all claims, demands, causes of action or other relief for damages for personal or bodily injury or property damage of any kind or nature whatsoever arising out of or in the course of participation in the Event. This agreement extends to all claims of or in the course of participation in the Event. This agreement extends to all claims of any kind whatsoever, foreseen or unforeseen, known or unknown. I will permit use of any photographs, video, or other record for lawful purposes and/or promotional purposes of event directors. If I am participating with a dog I do not own, I will not participate unless the dog's owner has read the entry form and this Agreement and executes it, making all the certifications and warranties contained in it, which I believe to be true. This Agreement shall be binding upon me, my child or ward, and my heirs, executors, administrators and assigns, or anyone else who may claim on their behalf.
